Our Fees

As an in-network provider for many Insurance Companies and Employee Assistance Programs, we have agreed to some discounted fees. A list of companies and programs we partner with is available via email request (please take note that Medicare is not accepted because of current legislation).

 

Ultimately, you are responsible for making sure that your insurance is active and accepted, and we encourage you to call and verify it before the 48-hour cancellation period ends. Our office will submit your claims directly with your permission and activate your benefits. However, even doing so, you will be responsible for any non-covered costs, such as co-pays or deductibles.

 

If you choose to be a private-pay patient, we accept cash and all major credit cards. We will bill your account for all outstanding balances. Reduced-fee services are available via our Affordable Service Plan Program.

The Affordable Service Plan
 
Sometimes our clients prefer not to bill through their insurance, or they do not have insurance that they can use at Strategies for Success for whatever reason. As an alternative to insurance, Strategies for Success offers the Affordable Service Plan (ASP) Membership.
 
We will do everything possible to work with your insurance plan or EAP benefit. However, at times, clients prefer not to bill through their insurance, or they do not have insurance that they can use at Strategies for Success for whatever reason. As an alternative to insurance, Strategies for Success offers the
Affordable Service Plan (ASP) Membership.
A membership fee of $59.00 per individual or $149.00 per family is a one-time charge for Access to our Affordable Service Plan Fee Schedule, when you are attending on a consistent basis.  If you have a lapse in services for 6 months or more, there will be a reduced fee to activate your membership again.
